Do I need a menopause specialist?

Standard guidance is often outdated, overly cautious, or misaligned with the latest research. Why isn’t a hormone lab test required before clinical care?

Hormone blood tests are not routinely required to initiate or monitor Hormone Replacement Therapy (HRT), especially for women experiencing menopausal symptoms. This is because treatment is based on clinical symptoms and medical history rather than lab values alone. Menopausal symptoms such as hot flashes, sleep disturbances, and mood changes are the primary indicators for HRT, and symptom relief is a more practical and reliable measure of effectiveness than achieving specific hormone levels.

During perimenopause, hormone levels — particularly estrogen and progesterone — fluctuate significantly, often varying not only day to day but even hour to hour. A single blood test offers only a snapshot in time and may not accurately reflect the broader hormonal picture or guide dosage decisions. Additionally, there is no medically defined “optimal” level of hormones during the menopausal transition, and individual experiences vary widely in both hormonal shifts and symptom severity.

That said, there are specific situations where blood testing can play a valuable role. For individuals under age 40 with symptoms suggestive of menopause, lab tests may help diagnose premature ovarian insufficiency (POI). Between ages 40 and 45, a follicle-stimulating hormone (FSH) test may be used when the diagnosis is unclear or to rule out other causes of symptoms. If testosterone is part of the HRT regimen, blood tests are important to monitor and maintain safe hormone levels. Additionally, if symptoms persist despite increased estrogen dosing, lab testing may be helpful to evaluate potential absorption issues.
